Complete the Application Checklist |
Complete the Application Checklist and make sure
you include all of the information needed to process your application.
You application package must include:
 A completed Working Wheels Application and statement of need.
A signed Credit Report Request and Release of Information Form.
A copy of your driver's license and your spouse's, if living in your
household.
Copies of your last 2 paycheck stubs.
A $10 non-refundable application fee to cover administrative costs
including pulling your credit report. A cashier's check or
money order is preferred.
A completed copy of this Application Checklist
